Instructions

The Fast-Track Cities Quality of Life Survey questionnaire asks questions about your health and wellbeing, as well as factors that can affect your quality of life. Please answer all the questions. Most questions require you select only one response or fill in the blank. For some questions, you may have the option of selecting multiple responses. In those instances, only select those that are most applicable to you or fill in the blanks. If you are unsure about which response(s) to give to a question, please choose the one that appears most appropriate.

There are no right or wrong answers; we are simply looking for your opinions and experiences.  Your answers will be kept absolutely confidential. No identifying information about you, such as your name or contact details, will be collected or revealed to anyone. 

The information you provide will be analyzed together with information from other respondents.  We will share and/or publish results from this survey using only overall summary data with information from all respondents combined.
